This privacy policy explains what information Veneers & Co. Dental Group collects through veneersco.com, how it is used, shared and protected, and the choices you have. Health information in your patient record is covered separately by our Notice of Privacy Practices. Questions about privacy can be directed to (818) 658-9285.

1. Who does this policy cover, and what does it not cover?
This policy covers information collected through veneersco.com (the "site") by Veneers & Co. Dental Group, the dental practice of Faraz Farahnik Inc., APC, at 16260 Ventura Blvd, Suite 730, Encino, CA 91436 ("we," "us").
- Covered: what you do on the site. The site does not currently offer online forms, photo upload or online scheduling; requests are made by phone or in person. We will update this policy before adding any such feature.
- Covered by our Notice of Privacy Practices instead: your dental records and other health information we keep as your dental provider, including anything you share with us that becomes part of your patient record. If this policy and the Notice of Privacy Practices ever differ about health information, the Notice of Privacy Practices controls.
- Not covered: websites we link to, such as financing companies (CareCredit, Cherry, Sunbit), Instagram and Google Maps. Their own privacy policies apply once you leave our site.

2. What information will the website collect?
We do not collect through the site: Social Security numbers, payment card numbers, insurance member IDs in open fields, or precise location.

How we use it: to answer your request and schedule care; to plan a consultation; to keep the site working and secure; to keep records the law requires; and to protect our rights and our patients' safety. We do not use your information for advertising, profiling or automated decisions about you.

3. How will health information sent through the site be protected?
- The site does not currently offer online forms, photo upload, online scheduling or video visits, so health information is not collected through the site. Requests are made by phone or in person.
- Please don't email us health information or photos. Call (818) 658-9285.
- We will update this policy before adding any online form, upload, scheduling or video feature. Any such feature that handles health information will use a provider that has signed a HIPAA business associate agreement with us.
- Once you become a patient, information you share with us becomes part of your patient record. HIPAA, California's Confidentiality of Medical Information Act (Civil Code §56 and following) and our Notice of Privacy Practices then apply.

4. Will the website use cookies, analytics or tracking?
What we never use: advertising pixels (such as Meta or TikTok), advertising or remarketing cookies, session replay or heatmaps, or third-party chat widgets. That applies to every page.
No analytics, tracking or third-party scripts. The site does not use analytics, advertising or tracking cookies, and it does not load third-party scripts on any page. The site itself does not set cookies. Our web host, Cloudflare, may use strictly necessary cookies or similar technology for security and bot protection.

We will update this policy before adding analytics or any other non-essential technology.
Global Privacy Control (GPC). If your browser sends a GPC signal, we treat it as a request to opt out of the sale and sharing of your personal information. We don't sell or share it in any case.
Do Not Track. Browser "Do Not Track" signals have no common standard. We don't track visitors across other websites in any case: we use no analytics or advertising trackers. We honor GPC as described above.
Other parties. We don't allow other companies to collect information about your activity across different websites through our site. If you follow a link to another site, such as Google Maps or Instagram, that site's own policies apply.

5. Will the practice sell or share personal information?
No. We do not sell personal information, and we do not "share" it for cross-context behavioral advertising (as California law defines those terms). We have not done so in the past 12 months. We do not knowingly sell or share the information of anyone under 16. We do not disclose personal information to others for their own direct-marketing purposes (Civil Code §1798.83).
We disclose information only to service providers that work for us under contract, and only as needed:

The site does not currently use online form, photo upload, scheduling, video or analytics providers.
We may also disclose information when the law requires it, to protect someone's safety, or as part of a sale or transfer of the practice, subject to the same protections.

6. How will virtual consultations be handled?
- Video visits are not currently offered through the site.
- If we offer them later, we will update this policy first, and we will use a platform whose provider has signed a business associate agreement with us.
- Before any first video visit, we would explain telehealth and ask for your consent, and document it (Business and Professions Code §2290.5).
- A video visit is for discussing your goals and options. It is not an examination. A diagnosis and treatment plan require an in-person exam in Encino. See our Terms of Use, section 7.

7. How will patient photos and testimonials be handled?
- No patient photo, video, name, initials, quote or treatment story appears on the site, our social media or our ads without the patient's signed written authorization. The authorization meets HIPAA (45 CFR §164.508) and California law (Civil Code §56.11), names each use the patient agrees to, has an end date, and can be cancelled. Use of a person's name or likeness is also covered by a separate consent under Civil Code §3344.
- Photos are not retouched. Hidden file information (location, date, camera) is removed, and file names never contain patient names.
- A patient can cancel at any time in writing to our Privacy Officer (section 10). We will remove the photos from our website and our own social media and advertising accounts within 30 days of receiving the request. Copies that others have already made can't be recalled.

8. How will information be kept secure, and for how long?
Security. HTTPS everywhere, security headers and access limited to staff who need it. The site has no online forms or uploads. No method is perfectly secure. If a breach affects your information, we will notify you as federal and California law require (HIPAA Breach Notification Rule; Civil Code §1798.82).

9. What about children?
The site is meant for adults and is not directed to children under 13. We do not knowingly collect personal information online from children under 13. A parent or legal guardian should make appointment requests for a minor. If you believe a child has sent us information, call (818) 658-9285 and we will delete it, unless it has become part of a patient record we must keep.

10. How will visitors exercise their privacy choices?
Patients. Your rights to see, copy, correct and get a list of disclosures of your dental records are explained in our Notice of Privacy Practices. Former patients of Dr. Farahnik's earlier practice can use the patient records page.

Website information: your California privacy rights. We offer these choices to every California visitor as our policy. Where the California Consumer Privacy Act applies to us, they are also your legal rights:
- Know what personal information we collected about you, where it came from, why, and who we disclosed it to; and get a copy.
- Delete personal information we collected from you, subject to legal exceptions (for example, records we must keep).
- Correct inaccurate personal information.
- Opt out of sale or sharing. We don't sell or share, and we honor GPC automatically.
- Limit the use of sensitive personal information. We use it only to provide the services you request.
- No discrimination or retaliation for using any of these rights.
These rights do not apply to medical information governed by California's Confidentiality of Medical Information Act or to protected health information governed by HIPAA (Civil Code §1798.146). That information is handled under our Notice of Privacy Practices.
How we respond. We will confirm receipt within 10 business days and respond within 45 calendar days. If we need more time (up to 45 more days), we'll tell you why. We will verify your identity by matching information you give us with what we hold, and we won't ask for more than we need. You may use an authorized agent; we may ask for proof of the agent's authority and confirm your identity with you directly.

11. How will changes to this policy be announced?
The effective date is at the top of this page. When we make a material change, we will update the effective date and post a notice of the change at the top of this page and in the site footer for at least 30 days. Earlier versions are available on request at (818) 658-9285.

12. Visitors outside California and the United States
The site is operated from California and its information is stored in the United States. People from other states and countries are welcome to contact us. We apply the same protections to everyone: nothing non-essential runs on the site, and we never sell or share personal information. By contacting us, you understand that your information will be processed in the United States under this policy and U.S. and California law.
